We want your first visit to be as smooth and comfortable as possible. To help us, please print off the two forms below and fill them out prior to your first appointment. Please either type or print neatly in pen so we can quickly get your information transferred into the computer.

 

 

 

 

 

Feel free to provide the forms to us with one of these methods:

1. Complete them and bring them to your appointment.

 

2. Complete them, scan them, and e-mail them as PDF files to staff@garrisonfamilydental.com
